2011 HG Survey

Thank you for your interest in our 2012-2015 survey. This is a survey to determine the effectiveness of medications/treatments for HG.

Any publication of the results will NOT include ANY information related to your identity. We will keep the individual responses strictly confidential.

Many women filling out our survey are pregnant and wish to complete it after they give birth. Others need to enter their answers during more than one session.
