2004-2005 FRS Survey

Thank you for your interest in our 2004-2005 survey. The survey will take about 5-15 minutes depending on how many pregnancies you have had. Our goal is to publish the results to increase awareness of issues related to HG and also use the results to determine the focus of future research projects.

Any publication of the results will NOT include ANY information related to your identity. We will keep the individual responses strictly confidential.

Many women filling out our survey are pregnant and wish to complete it after they give birth. Others need to enter their answers during more than one session. For these reasons, we ask that you sign in so you can make updates or additions in the future. We respect your privacy and will keep all data confidential.
