Where can I find some?

I've been wanting to put out more info on HG. Like others, when I actually had it, my midwife the first time around didn't acknowledge it at all. I was diagnosed with it suggestively at the ER with a print out giving a one line sentence about HG meaning "Excessive nausea and vomitting during pregnancy"... or you know, something short and sweet to that effect.
So, I was wondering if there's any print outs here on the site available to us that we can give out, or leave at our gyno/ob/midwife/gp offices?
I just looked back on several other threads here and saw suggestions of brochures and business card styles, and I love both... but are there any actually out there?
If there aren't any, have you made your own, and if you haven't made your own but someone made them available for you to print, would you use them? I'm fairly good at designing things on the computer and would love to help out making up something if there isn't anything already. If there is, I'm still looking for it, and could someone point the way for me?

In addition to the survival guide, there is also a "What is HG?" informational brochure, and one titled "When a Loved One has HG" that is great for handing out to friends and family of HG sufferers.
